This guide explains how to create and use the questionnaire activity.

What is the Questionnaire activity?

A questionnaire is an opportunity for students to provide staff with feedback on anything from lectures to entire modules. A questionnaire can be identified by an icon of a page with two ticks.

Creating the Questionnaire activity

Navigate to the module / course page that you want to add the Questionnaire activity to and ensure that you have editing enabled by clicking the "Turn editing on" button.

Scroll down to the section on the page that you want to add the activity to and select 'Add an activity or resource' from the bottom right of that section.

From the pop-up window click on 'Questionnaire'.

Next you will need to provide a name for the questionnaire and a description, the latter could be used to explain how questionnaires work and what the results will be used for.

The first group of settings under "Timing" allow you to set when the questionnaire is open for submissions. If the open date checkbox is not selected, the questionnaire will be open immediately.

The "Response options" offer a number of settings worth exploring. The most important of these are arguably "Respondent Type" which determines if submissions are anonymous, and "Students can view ALL responses" which determines if students can view responses.

Many of the options can be changed using the dropdown menus.


Click 'Save and display' to successfully create a questionnaire activity.


Adding questions

When you create a new questionnaire, it will be blank. You can add questions by clicking the 'Add questions' link.

There are a range of different types of questions that you can add to a questionnaire, in this example we will add a check box by selecting 'Check boxes' from the drop down and click the 'Add selected question type' button.

You will then be taken to the question settings window, this will look different depending on the type of question you select.

In the example above we ask users to select their favourite type of dog from a list of three that we provide in the "Possible answers" box. Note that the "Question Name" field is not where you enter the question, but rather the "Question Text" box.


By clicking "Save changes" you will be returned to the questionnaire you are currently editing and able to add more questions. To preview the questionnaire, select "Preview" from the navigation bar.

Editing and deleting questions

To view the questions within a questionnaire, first click the title on the right hand side of the questionnaire icon. By clicking the cog icon on the right you are able to select "Questions" from the drop down menu.

From this screen you can view all of the questions within the quiz. To delete a question, select the 'x' icon.

A window will check whether or not you want to delete the question, select 'Yes'.

You can edit a question by selecting the cog icon from the first screenshot in this section. You will then be presented with the edit question settings window shown below.
