This guide explains how to create and use the questionnaire activity.

What is the Questionnaire activity?

The Questionnaire activity allows staff to survey students using a range of questions to get student feedback about a module or specific activities.


Creating the Questionnaire activity

Navigate to the module / course page that you want to add the Questionnaire activity to and ensure that you have editing enabled by clicking the "Edit mode" toggle.

Scroll down to the section on the page that you want to add the activity to and select 'Add an activity or resource'.

From the pop-up window click on 'Questionnaire'.

Enter a name and description for the questionnaire. The description area could be used to explain the purpose of the questionnaire and what the results will be used for.

Availability - Click the 'Enable' checkboxes and select allow responses from and until dates and times. If the Enable checkboxes are left unchecked, the questionnaire will be open immediately.  

Response options - This section has several settings for you to explore. A selection of the settings are detailed below and for others you can find out more by clicking the relevant 'question mark' icons when creating a questionnaire.

  • Type - select whether users can respond once, daily, weekly, monthly or an unlimited number of times.
  • Respondent Type - Show student names or hide identities by selecting anonymous.
  • Students can view ALL responses - after answering the questionnaire, after the questionnaire is closed, always or never.
  • Save/resume answers - allows students to save their responses and resume from the same point at a later date.

Click 'Save and display' to successfully create your questionnaire activity.


Adding questions

When you create a new questionnaire, it will be blank. You can add questions by clicking the 'Add questions' button.

There are a range of different types to select from. In this example we will add a check box by selecting 'Check boxes' from the drop down menu and clicking the 'Add selected question type' button. 

Enter the question name (this can be the same as the question text) and indicate whether a response is required.

Enter your question into the question text area, then enter your possible answers separated by commas.

Click 'Save changes' to return to the Questions page and repeat the steps to add more questions to your questionnaire.

To preview the questionnaire, click 'More' in the top navigation bar, then select 'Preview'.

You will then see a preview of your questionnaire.


Editing and deleting questions

Click the 'Questions' link in the top navigation bar.

This will open the Questions page and display all questions that you have added to the Questionnaire.

To edit a question, locate it and click the 'Cog' icon which will open the question in Edit mode.

Edit the question – e.g., change question text, settings etc.

Then click ‘Save changes’.

To delete a question, locate it and click the 'X' icon.

A pop up will then open for you to confirm the deletion. Click 'Yes' to proceed.
